Contract

This Service Agreement (“Agreement”) is a required legal document between you (“Client”) and CityRN, LLC (“CityRN”). In order for CityRN to provide nursing services, you must review, complete, and sign this Agreement.

To finalize the Agreement, please fill in the required information and submit the form. Your electronic signature will be considered legally binding and enforceable, just as if you had signed a paper copy.

Services cannot be scheduled or confirmed until the signed Agreement has been received.
